The researcher is required to forward the application to the Ethics Committee Presidency at least three weeks before the start of the project and submit the signed documents to the Secretariat of the Board.

The following documents are submitted by the responsible researcher;

1- Document which contains all information relating to research mentioned in the application 

2- Contained documents in the forms section,

* Application Petition 

* Application form

* Voluntary Participation Form (if applicable) 

* Parental Consent Form (if applicable)

* Commitment

3- Questionnaires and contents to be used in survey, interviewing, shooting, focus groups and such applications, 

4- Legal permits for all kinds of surveys, interviews, shooting, focus groups and such applications,

5- CVs of all researchers
