Staff Teaching Mobility Forms
The following documents and forms are required for staff teaching mobility. To ensure your documents are complete and accurate, please carefully review the Staff Monitoring Forms (files 1 and 5) first! These files will guide you through the process of preparing your forms.
Forms to be prepared by academic staff who are eligible to participate:
1- Staff Monitoring Form
2- Staff Mobility Agreement for Teaching
3- Staff University Contract
Document to be completed if the staff wishes to request special needs: Staff Special Needs Request Form

7- Online EU Survey:
After the above documents are prepared and submitted to the office, your registration will be completed in the European Commission system. Following this online registration, a survey will be automatically sent to your email address. After completing this survey, you must send an email to erasmus@erzurum.edu.tr confirming your completion.
Personnel wishing to waive their rights must fill out the waiver form and send it to erasmus@erzurum.edu.tr. Click here for the Waiver Form.
